Not every certificate is the same document. The type you need depends entirely on your reason for needing it.
A sick leave certificate is what tourists need most often. It documents that you were genuinely ill during a specific period. Airlines, travel insurers, and home employers all accept it. For international use, it needs to be in English, include the diagnosis, cover the dates you were unwell, and carry the physician’s name, licence number, and the clinic’s official stamp.
A fitness certificate confirms you are healthy enough to do something specific: fly, dive, return to work, or take part in a sport. Divers heading back out after an ear infection sometimes need one before a liveaboard operator will clear them. The exam takes a few minutes and the certificate follows on the same visit.
A work permit medical certificate is a separate format required by Thai law. It follows the Department of Employment’s set structure and screens for a specific list of prohibited conditions. Expats applying or reapplying for a Thai work permit need this one, and it must be issued by a registered clinic with a licensed physician.
A general health certificate covers overall health status. Employers, schools, visa applications, and some sporting bodies ask for this. It is broader than a sick leave note and more detailed than a fitness certificate.
What Makes a Medical Certificate Valid in Thailand
This is where documents get rejected, and it is avoidable.
A valid Thai medical certificate needs a licensed physician’s signature, not a nurse or assistant. It needs the clinic’s official seal, which is a physical or registered stamp, not a printed logo. The issuing clinic must be a registered medical facility.
For local use, Thai language is fine. For international use, English is essential. A bilingual certificate covers both. Your insurer in Germany or the airline’s claims team in London will not process a Thai-only document.
If the certificate is going to a foreign travel insurer or airline, ask for an ICD-10 diagnosis code to be included. This is the international system for classifying illnesses, and most insurers now require it for claims above a certain value. Medical Certificates for Tourists — Sick Leave, Insurance Claims, and Flight Changes
This is the situation I treat most often. You are mid-holiday, something has gone wrong, and you cannot fly. You need to change dates. Your insurer wants documentation before they will reimburse anything.

One practical point: check your travel insurance policy’s documentation requirements before you leave the clinic. Some policies require the certificate to be issued on the day you first had symptoms, not several days later. If you are already past that window, come in anyway. I can document when symptoms started based on your clinical history, but earlier is always easier.
Direct billing to travel insurance is not always available for a certificate consultation alone, so keep your receipt. Your insurer will want it alongside the certificate.

Work Permit Medical Certificates in Phuket — What Thai Law Now Requires
If your work permit is due for renewal, or you are applying for the first time, you need this specific certificate. It is not the same as a sick leave note or a general health check.
The Thai Department of Employment requires a certificate from a licensed physician at a registered clinic. As of January 2026, the e-Work Permit system is fully digital, and the certificate information must meet the format required for digital submission. Not every clinic is set up for this, so confirm before you sit down.
The examination screens for what Thai law calls prohibited diseases. The current list has three conditions. You will not be told there is a problem unless the examination finds one. For the vast majority of applicants, this is a short physical check and a certificate issued the same day.
Bring your passport and a passport-style photo. Some employers also require the certificate to be issued within 30 days of your work permit application date, so time it accordingly.
