Gonorrhea is a bacterial infection spread through unprotected vaginal, anal, or oral sex. It often causes no symptoms at all, which is why testing after unprotected sex matters more than waiting to feel sick. A single visit to a walk-in clinic in Phuket is enough to get tested and treated the same day.

You had a night out in Patong. Maybe more than one. You are a few days into your trip, or already at the airport, and something feels off. A burning sensation when you urinate. A discharge you did not have before. Or nothing at all, but a nagging awareness that the condom did not happen. You are Googling from your hotel room trying to decide whether to act now or wait until you are home.

Do not wait. Gonorrhea is curable with the right antibiotics, but only if you get them before complications develop. This article covers what to watch for, how testing works in Phuket, and why the treatment picture here has some real differences from what you may expect at home.

What Gonorrhea Symptoms Actually Look Like

Most people picture one thing: genital discharge and pain when urinating. That picture is incomplete, and in Phuket that gap matters.

For men, the classic signs are a yellow or green discharge from the penis and a burning feeling when urinating. These can appear anywhere from one day to two weeks after exposure. Some men have no symptoms at all.

For women, the infection is even more likely to be silent. When symptoms do appear, they include unusual vaginal discharge, pain during sex, or bleeding between periods. Many women find out only after a partner tests positive, or after complications have already developed.

Throat infections are common after oral sex and usually cause no symptoms. When they do, they look like an ordinary sore throat, which is why so many go undiagnosed. Rectal infections can cause discharge, itching, or discomfort, but again, often nothing at all.

If you had unprotected oral, anal, or genital sex, do not assume a clear throat means you are clear.

How Long Before Gonorrhea Symptoms Appear

Symptoms in men, when they appear, usually show up within two to five days of exposure. In women, symptoms take longer and are far less reliable as a signal. For throat and rectal infections, you could carry the infection for weeks without any sign at all.

I tell patients this every time: the absence of symptoms is not a result. It is just silence. The only way to know is to test.

Why Gonorrhea in Phuket Carries Extra Risk

Testing demand rises sharply after weekends in Patong. Many visitors come in after unprotected sex with a partner whose sexual history they do not know. Partner turnover is high, alcohol lowers judgment about protection, and the infection moves quickly through a population that is constantly rotating.

That risk profile alone is reason enough to take gonorrhea seriously here. But there is a second factor that makes Phuket different from getting this infection at home.

Drug-Resistant Gonorrhea in Thailand

Antibiotic resistance in gonorrhea is a real and growing problem in Thailand. A ceftriaxone-resistant strain was confirmed in Phuket Province in early 2025 through national Thai surveillance data. This is not a reason to panic, but it is a reason to get proper treatment rather than guessing.

The bigger concern I see clinically is self-treatment. Antibiotics are easy to buy over the counter at pharmacies across Phuket. Some patients take whatever is available before coming to see me. That can suppress symptoms without clearing the infection, and it speeds up resistance. If you have already taken pharmacy antibiotics, tell the doctor. It changes how we approach your case.

Treatment follows Thai and WHO first-line guidelines. In most cases this means a single intramuscular injection of ceftriaxone, usually combined with doxycycline to cover chlamydia at the same time. A test first, then the right drug at the right dose. Not a pharmacy guess.

How Gonorrhea Testing Works in Phuket

The two main ways to test are a urine sample and a swab. Which one depends on where the potential infection is.

A urine test works well for genital infections. For throat or rectal infections, a swab from the relevant site is needed. A urine test alone will miss these. If you had oral or anal sex, say so. It determines which sites we test.

We use NAAT testing at Takecare Clinic Phuket, which is the most accurate method available for each site tested. Before your urine sample, do not urinate for at least one hour. That is the main preparation. You do not need to fast, and you do not need to explain anything to anyone else.

What Happens During and After Your Visit

Walk in, no appointment needed. You tell me what happened. I ask a few direct questions about timing, the type of sex, any symptoms, and any antibiotics already taken. Then we test.

If your result is positive, or if symptoms and exposure history make gonorrhea highly likely while results are pending, treatment can start the same visit. The ceftriaxone injection takes about a minute. Doxycycline capsules, if needed, go with you. Abstain from sex until treatment is complete and symptoms have resolved. Retest around two weeks later to confirm the infection has cleared.

Partner notification is part of the process. If your partner is still on the island, they need to be tested. If they have already left Phuket and returned home, most countries have confidential partner notification services. I can advise on this during your consultation. It is not an awkward conversation. It is just good care.

Complications of Untreated Gonorrhea

A two-week holiday can easily become the window in which untreated gonorrhea causes real damage.

In men, the infection can spread and cause epididymitis, a painful condition affecting the tube behind the testicle that can affect fertility without treatment. In women, it can move into the uterus and fallopian tubes, causing pelvic inflammatory disease. That is one of the more common preventable causes of chronic pelvic pain and infertility I see in the context of delayed diagnosis.

There is also the HIV connection. Active gonorrhea increases the risk of acquiring HIV during unprotected sex. If there is any possibility of HIV exposure at the same encounter, that conversation needs to happen at the same visit. PEP, the medication that can prevent HIV from taking hold after exposure, must start within 72 hours. Same-day access to a Patong walk-in clinic is not just a convenience for PEP. It is the whole point.

On rare occasions, untreated gonorrhea spreads into the bloodstream. Joint pain, skin lesions, and fever in the days after a potential exposure warrant immediate assessment, not a wait-and-see approach.

Gonorrhea and Chlamydia Often Arrive Together

These two infections look almost identical and co-occur frequently. If you are tested for one without the other, you may clear gonorrhea and remain infected with chlamydia, or the reverse. Current guidelines recommend treating for both when chlamydia has not been excluded by testing, which is why doxycycline is usually part of the same visit.

NAAT testing tells the two apart accurately. Our STI screening at Takecare Clinic Phuket covers both in a single visit so nothing gets missed.

If you had unprotected sex in Phuket and want to get tested for gonorrhea or other STIs, Takecare Clinic Phuket is open daily with no appointment needed. Our English-speaking doctor in Patong can test, diagnose, and treat in a single visit with full confidentiality. We also cover PEP for potential HIV exposure, chlamydia co-testing, and ongoing sexual health care for expats and long-stay visitors across the island, from Bang Tao to Rawai. Walk in anytime.

Can I get tested and treated for gonorrhea the same day in Phuket without an appointment?

Yes. Takecare Clinic Phuket is a walk-in clinic in Patong with no appointment needed and an English-speaking doctor available daily. Urine and swab testing is done on-site. If the result is positive, or if clinical suspicion is high enough based on your symptoms and exposure history, the ceftriaxone injection can be given the same visit without waiting for results.

I had unprotected sex in Patong but feel completely fine. Do I still need a gonorrhea test?

Yes, and this is exactly the situation where testing matters most. A large proportion of gonorrhea infections, particularly in women and at throat or rectal sites, cause no symptoms at all. Feeling fine does not mean you are clear. Testing around two weeks after exposure gives a reliable result, and catching it early means a straightforward single-visit treatment rather than managing complications later.

Is gonorrhea harder to treat in Thailand than back home?

The standard first-line treatment, a ceftriaxone injection, still works in the large majority of cases. The concern is that resistant strains have been confirmed in Thailand including in Phuket Province, and that over-the-counter self-treatment is common here and can mask symptoms without clearing the infection. Always see a doctor first rather than treating yourself with pharmacy antibiotics.

My partner already flew home after our time in Phuket. What do I do about telling them?

Your partner needs to be tested and treated even if they have no symptoms. Most countries have confidential partner notification services, and some clinics offer anonymous digital notification tools. I can advise on the best approach for your partner’s home country during your consultation at Takecare Clinic Phuket.

Could it be chlamydia instead of gonorrhea, and does it matter which one I have?

It matters, because the two infections co-occur often enough that testing for both at the same time is standard practice, and the treatment regimens differ slightly. NAAT testing tells them apart accurately. Testing for one and assuming the other is absent is not good practice, which is why our screening covers both in the same visit.

I did a liveaboard trip out of Chalong and had sex at one of the stops. Is my risk any different from someone who stayed in Patong?

The infection risk is the same wherever unprotected sex occurred. What changes is the testing window. If you are still on a multi-day trip and symptoms appear, do not wait until you are back in Chalong or Phuket Town. Come in as soon as the trip ends, tell me when the exposure happened, and we test and treat from there.
