Gonorrhea treatment in Phuket is straightforward: one intramuscular ceftriaxone injection, given the same day you are tested. Most urethral symptoms start to ease within three to five days. At Takecare Clinic Phuket, you can walk in without an appointment, get tested and treated in a single visit, and leave with clear guidance on what to do next.
Two nights ago you were in Patong. This morning there is discharge you cannot explain, or urination that burns in a way it did not yesterday. Or maybe you feel nothing at all, but the thought is there and it will not leave. All three situations bring people through my door, and all three can be sorted the same day.
Gonorrhea is one of the most common bacterial STIs I diagnose in Phuket. It is also curable with a single injection. What most patients do not know when they walk in is what happens after that injection, whether they need a follow-up test, and what to do about a partner they may not be able to reach. This article covers all of that.
Symptoms Worth Acting On — and the Ones You Might Miss
The classic signs in men are burning on urination and a thick yellow or green discharge from the tip of the penis. In women, symptoms are quieter. There might be unusual vaginal discharge or pelvic discomfort, but many women have no symptoms at all.
Asymptomatic gonorrhea is real and common. I see patients who feel completely fine but test positive because a partner told them to get checked. If you had unprotected sex and there is any doubt, that is reason enough to come in.
It Is Not Always Genital — Sites of Infection That Get Missed
This is the part most patients do not expect. Oral sex can deposit the bacteria in your throat. Pharyngeal gonorrhea almost never causes symptoms. You would not know you had it. Rectal gonorrhea can cause discharge or itching, but it is also often completely silent.
If testing only covers urine, throat and rectal infections stay invisible. At Takecare Clinic Phuket, I take the history first and collect samples from every relevant site in one visit. One visit, all sites covered.
Why the Gonorrhea Injection Is Non-Negotiable Now
Patients sometimes ask if they can just take a tablet and skip the injection. The honest answer is no.
Fluoroquinolone antibiotics such as ciprofloxacin no longer work reliably against gonorrhea in Thailand. Resistance rates here are high enough that these drugs are not recommended. Oral cefixime is weaker than injected ceftriaxone and performs particularly poorly against throat infections. The injection reaches concentrations in the tissues that oral treatment simply cannot match.
The injection takes seconds. It is given in the muscle, usually the upper arm or buttock. Most patients are surprised by how straightforward it is.
What to Expect in the Days After Your Injection
This is the question I get asked most.
Days one and two: the antibiotic is working, but you are unlikely to notice much change yet. Do not read that as the treatment failing. Days three to five: discharge typically starts to reduce and burning on urination begins to ease. By day seven, most urethral symptoms should be significantly better.
Throat infections take longer. Pharyngeal gonorrhea can take one to two weeks to fully resolve. If you still have mild urethral discomfort at the one-week mark but discharge has cleared, you may have post-gonococcal urethritis, a low-grade irritation that can linger after the gonorrhea itself is gone. It is not a treatment failure. If nothing has improved at all by two weeks, come back for reassessment.
You should not have sex, including oral sex, until symptoms have fully resolved and at least seven days have passed since your injection.
Do You Need a Follow-Up Test After Gonorrhea Treatment?
The answer depends on where your infection was.
For uncomplicated urethral or genital gonorrhea, a routine test of cure is not required by current guidelines if you received the correct injection and have no ongoing symptoms. A reinfection screen at three months is recommended, because the main reason gonorrhea comes back is an untreated partner, not antibiotic failure.
Pharyngeal gonorrhea is different. A throat swab test of cure is recommended at two weeks minimum. Testing earlier can trigger a false positive, because residual bacterial DNA stays detectable even after the infection is cleared. At Takecare Clinic Phuket, I can arrange both the initial test of cure and the three-month screen, walk-in, no appointment needed. You can read more about what to expect after STD treatment in Phuket on our testing and follow-up page.
Partner Notification — The Part Nobody Wants to Think About
I understand this is the difficult conversation. But an untreated partner means reinfection, and reinfection means starting over. This back-and-forth is genuinely the most common reason gonorrhea comes back.
Current guidelines recommend notifying all partners from the past 60 days. If your partner is still in Phuket, bring them in. I can treat a partner without requiring their own positive test first, which is the standard approach called expedited partner therapy. They do not need to explain anything. They just need to come in.
For anonymous encounters, anonymous notification tools exist through public health services in many countries. I can point you toward those during your consultation.
Testing, Confidentiality, and What the Visit Actually Looks Like
In a place like Patong, confidentiality matters in a way it might not in a large city. You might be staying in the same hotel as people you know. I hear this concern regularly.
Walk-in STD testing and treatment at Takecare Clinic Phuket is private. You do not give a reason at reception. You do not need an appointment. You see a doctor, and everything discussed in that room stays there.
The visit itself is straightforward. We talk through your symptoms and history. I collect the right samples, urine, swab, or both. If your picture is consistent with gonorrhea and you want to be treated the same day, the injection can be given before lab results come back. The NAAT test, a highly accurate urine or swab test for gonorrhea, typically confirms results within one to two days. I also check for chlamydia in the same visit, because the two travel together often enough that testing for both is standard practice. If you are thinking about broader STD testing in Phuket, we can cover that in one visit as well.
If you think you have gonorrhea, or you just want to be sure after an unprotected encounter, walk into Takecare Clinic Phuket in Patong. No appointment needed. English-speaking physician care. We test, treat, and advise in a single visit, including partner treatment if needed.
How quickly will my symptoms improve after the gonorrhea injection?
Most patients notice discharge reducing between days three and five, with burning on urination easing around the same time. By day seven, urethral symptoms should be significantly better. Mild lingering discomfort after that can be post-gonococcal urethritis, not treatment failure, but if nothing has changed at two weeks, come back for reassessment.
Do I need to come back for another test after gonorrhea treatment?
For genital gonorrhea with no complications, a test of cure is not routinely required, but a reinfection screen at three months is recommended. For pharyngeal gonorrhea, a throat swab at two weeks is needed. Do not retest before two weeks, as residual DNA can trigger a false positive.
I had a brief encounter in Patong and cannot contact the person — what do I do about partner notification?
Guidelines cover anyone you had contact with in the past 60 days. For anonymous partners, notification services exist through public health programs in most countries, and I can point you toward those in the consultation. If the person is still on the island, bring them in and I can treat them without requiring a positive test on their end.
Can gonorrhea infect my throat or rectum, and does the treatment change?
Yes, and both sites are commonly missed because they often cause no symptoms. The same ceftriaxone injection treats all sites, but I need a throat swab or rectal swab in addition to urine to test those sites accurately. Pharyngeal gonorrhea also requires a test of cure at two weeks, which uncomplicated genital infections do not.
Why can I not just take a tablet instead of having the injection?
Fluoroquinolone antibiotics have high resistance rates in Thailand and are no longer recommended for gonorrhea here. Oral cefixime is weaker than injected ceftriaxone and particularly unreliable for throat infections. The injection achieves tissue concentrations that oral treatment cannot, which is why it is the current global standard.
I feel fine and have no symptoms — do I still need to get tested after unprotected sex in Phuket?
Yes. Asymptomatic gonorrhea is common, especially in women and at throat and rectal sites. Feeling fine does not rule out infection. If you had unprotected sex and there is any doubt, come in. The test is straightforward and the whole visit takes under an hour.
