GLP-1 medications like semaglutide and tirzepatide are given as subcutaneous injections, meaning just under the skin, usually into the abdomen, outer thigh, or upper arm. Most people inject once a week using a pre-filled pen or a vial and syringe. The technique is straightforward once you have done it once, but getting it right from the start matters for both safety and how well the medication works.
GLP-1 medications are not suitable for everyone. They are contraindicated in people with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. If you are unsure whether these apply to you, speak with a physician before starting. Your first GLP-1 pen is sitting on the bathroom shelf of your Kata villa. Or your compounded semaglutide vials just arrived and you are staring at a syringe you have never touched before. The prescription is sorted. The harder part, for most people, is actually picking up the needle.
That hesitation is normal. First-time injection anxiety is common, whether someone is a long-stay expat starting a weight management programme in Chalong or a visitor who began treatment before flying out and needs to continue here. This guide walks you through the full technique, covers what differs between a pen and a vial, addresses the storage issues specific to Phuket’s heat, and tells you when to come in rather than guess.

What You Need Before Your First Injection
For a pre-filled auto-pen like an Ozempic or Mounjaro pen, you need the pen itself, the correct needle attachment (usually included or supplied by your prescriber), and an alcohol swab. That is all.
For a compounded GLP-1 vial, you need the vial, the correct insulin syringe with the right gauge needle as specified by whoever prescribed it, alcohol swabs, and a sharps container. Do not use a plastic bottle or zip-lock bag for used needles. A proper travel sharps container is small, inexpensive, and available at most pharmacies across Phuket.
Keep everything in one place, a small toiletry bag works well. Keep it in your room, not in a beach bag or car. Storage reasons are covered in the next section.
Storing Your GLP-1 Medication in Phuket’s Heat
Average temperatures in Phuket sit around 32°C, and humidity is high year-round. A medication left in a beach bag, a parked car, or a hotel room with the air conditioning switched off is at real risk of heat damage.
GLP-1 medications generally require refrigeration before first use, between 2 and 8°C. Once opened or in use, some products tolerate a limited period at room temperature. The exact tolerance varies by product, so check the label or insert that came with your specific medication. If your pen or vial has been stored above the stated temperature, do not assume it is still effective. Degraded medication can look completely normal.
Store your medication in the minibar fridge if your room has one. If not, ask the hotel about refrigerator access. Never freeze it. For short transfers between areas, a small insulated cooling pouch with an ice pack wrapped in a cloth works well. Do not place the pen or vial directly against ice.
If your medication looks cloudy, has visible particles, or has changed colour from when you first opened it, do not inject it. Walk in to Takecare Clinic Phuket and a physician can help you assess whether it is still safe to use before your next dose.

How to Inject GLP-1: Step by Step
Using a Pre-Filled Pen
Wash your hands first. Take the pen out of the fridge and let it come to room temperature for a few minutes. Injecting cold medication is not dangerous, but it can sting more.
Choose your injection site. The abdomen, at least 5 centimetres (about 2 inches) from the navel, is the most common starting point. The outer thigh works well too. Attach the needle, remove both caps, and dial to your prescribed dose. Pinch a small fold of skin, press the pen firmly against it at a 90-degree angle, and press the button. Hold it in place for the count your pen instructions specify before removing. Do not rub the site.
Remove the needle, replace the pen cap, and put the used needle in your sharps container straight away.
Using a Compounded Vial and Syringe
Wash your hands, choose your site, and wipe the top of the vial and the injection site with separate alcohol swabs. Let both dry. Draw air into the syringe equal to your prescribed dose, inject that air into the vial, then draw out your dose. Remove any visible air bubbles by gently flicking the syringe and pushing them out.
Pinch the skin, insert the needle at 90 degrees, and slowly depress the plunger. For very lean individuals, a 45-degree angle may reduce the risk of hitting muscle rather than fat. Hold for a few seconds, withdraw, and dispose of the needle in your sharps container straight away.
The vial-and-syringe method has more steps than a pen. If you are doing this for the first time and feel uncertain, a supervised demonstration is a reasonable thing to ask for.
Injection Site Rotation and Why It Matters
Using the same spot every week causes lipohypertrophy, a build-up of fatty tissue that forms a firm lump under the skin. Injecting into these lumps reduces how well the medication absorbs, which can make treatment less effective without you realising it.
Rotate your sites deliberately. If you use the abdomen, move the injection point by at least a few centimetres each week and alternate left and right sides. You can also rotate between abdomen, outer thigh, and upper arm across different weeks. A rough mental note or a written log both work.
In Phuket’s heat and humidity, site reactions can develop a little faster than patients from cooler climates expect. A small red mark or mild bruising after injection is normal and usually settles within a day or two.
Injection Site Reactions: What Is Normal and What Needs a Doctor
Mild redness, minor bruising, and slight tenderness are common in the first few weeks. These are not a reason to stop.
What does need a clinical review: a hard lump that does not go away, spreading redness or warmth around the site, swelling that increases over hours, or any sign of skin infection. In a warm, humid environment like Phuket, skin infections can move faster than patients used to a temperate climate would expect. Do not wait a week to see if it resolves on its own if it looks infected.

What to Do If You Miss a Dose While Travelling
GLP-1 medications are typically given once a week. If you miss your scheduled dose and remember on the same day, inject as soon as you remember. If several days have passed, check the product-specific instructions. For semaglutide products, the label allows the missed dose to be given up to 5 days after the scheduled day. For tirzepatide products, the label allows up to 4 days. If the window stated on your label has passed, skip the missed dose and return to your regular schedule the following week. Do not take two doses to make up for it.
Time zone changes and irregular travel schedules can make this confusing. If you are unsure whether you are within the window, the safest option is to skip and restart next week. Check the product-specific instructions that came with your medication, as the exact missed-dose window varies between products.
When to Get Your First Injection Supervised
Most people can learn injection technique from clear written instructions. But some patients do better with a physician or nurse walking them through the first dose in person, and there is no reason to feel awkward about asking.
If you have never self-injected before, or if you are using a compounded vial and syringe rather than a pre-filled pen, a supervised first injection removes a lot of uncertainty. You see the correct technique demonstrated, ask questions in real time, and leave feeling confident rather than anxious.
At Takecare Clinic Phuket, walk-in appointments mean you do not need to plan ahead. Come in before your first injection and one of our English-speaking physicians can demonstrate the technique, confirm your dose preparation, and check your storage setup. If you need a prescription refill or documentation for travelling with your medication through Thailand, we can help with that during the same visit via our prescription refill service in Phuket.
For a broader look at your health while managing a GLP-1 programme, our health check-up service in Phuket can provide a useful baseline or follow-up alongside treatment.
