Fit-to-Work Certificate in Phuket: What It Covers and When You Need One

Your work permit renewal is due in two weeks. Your HR contact just sent a message asking for a fit-to-work certificate, and you are not sure whether the health check you had six months ago still counts. This is one of the most common administrative headaches I see among Phuket’s working expat population, whether you are teaching in Phuket Town, running dive trips out of Chalong Bay, or working a resort job in Kamala.

The process is straightforward once you know what is actually required. The confusion comes from not knowing what the certificate covers, how long it stays valid, or where to get one that Thai immigration will accept. This article answers all of that.

What a Fit-to-Work Certificate Actually Is

This is not a general sick note. A general medical certificate looks backward: it documents an illness you already have or had. A fit-to-work certificate looks forward. It is a signed statement from a licensed physician that you are medically cleared to perform your job duties.

Thai immigration and most Phuket employers treat these as completely different documents. Showing up to a work permit application with a sick note instead of a clearance certificate will cost you time and another trip to a clinic. Know which one you are asking for before you walk in.

Is a Fit-to-Work Certificate a Legal Requirement in Thailand?

For most foreign nationals applying for or renewing a Thai work permit, yes. Under the Foreigners’ Working Management Emergency Decree, a medical certificate from a licensed Thai physician is a required document in the application package submitted through the e-Work Permit portal.

Some employers also request this certificate independently of immigration, as a condition of employment or return to work after illness. In that case, the requirement comes from your contract or your employer’s HR policy, not Thai immigration law. Both situations produce the same kind of document, but the timing requirements differ.

The Six Prohibited Diseases Thai Law Requires Screening For

This is the part most clinic pages leave out, and it matters. Thai law requires the examining physician to screen for six specific conditions before issuing a work permit medical certificate.

Those six are leprosy, advanced pulmonary tuberculosis, drug addiction, chronic alcoholism, elephantiasis, and third-stage syphilis. A blood test for syphilis is included in the examination. The doctor will also assess for physical disability and any signs of mental illness that would affect your ability to work.

For the vast majority of people, none of these conditions are present and the certificate is issued the same day. The screening exists as a legal formality, not as a barrier.

The 30-Day Validity Rule

This is the timing mistake I see expats make most often. The certificate must be issued within 30 days of the date you submit your work permit application or renewal. Not 60 days. Not three months.

If you book your appointment too early and your application is then delayed for any reason, your certificate expires and you need to start again. Book your appointment as close to your planned submission date as possible.

Who Needs One Beyond Work Permit Applications

Work permit applicants are the most obvious group, but not the only one. In Phuket’s working environment, I see fit-to-work certificates requested in a few other situations regularly.

Hospitality and food service staff returning after a gastrointestinal illness often need employer clearance before going back to a kitchen or front-of-house role. Dive instructors returning after ear, sinus, or decompression issues may need a clearance note before going back in the water. Teachers at international schools sometimes need documentation after a significant illness or injury. If your employer’s HR policy requires this and you are unsure, ask them directly, then come in.

What the Examination Involves

The appointment is not complicated. You will have a brief physical check, the syphilis blood test, and a review for any signs of the six prohibited conditions. The whole visit typically takes under 30 minutes.

What Makes the Certificate Legally Valid in Thailand

The certificate must be signed by a physician registered with the Thai Medical Council. A certificate from an unregistered provider or a clinic without a licensed Thai doctor on staff will not be accepted by the Department of Employment.
