What a hangover actually is
A hangover is the body’s reaction to a large amount of alcohol once blood alcohol levels fall. Several things happen at once. Alcohol suppresses a hormone called vasopressin that normally tells the kidneys to hold on to water, so you pass more urine and become dehydrated. It irritates the stomach lining, which causes nausea, vomiting and burning pain. It disrupts deep sleep, lowers blood sugar in some people, and your body breaks it down into acetaldehyde, a toxic by-product linked to headache and feeling awful. Darker drinks such as whisky, brandy and red wine contain more congeners, other chemicals that make hangovers worse.
In Phuket, heat and sweating add to the fluid loss, and long nights out often mean little food and no water. That combination is why the dehydration part of a tropical hangover is often worse than the same night at home.
What is in our hangover drip, and what it can and cannot do
Our hangover recovery drip is typically around 1 litre of IV rehydration fluid with electrolytes, B-complex vitamins and vitamin C, plus anti-nausea and acid-reducing medicine when the doctor decides you need them. Commonly used anti-nausea medicines include ondansetron or metoclopramide. The session usually takes 45 to 60 minutes, in the clinic or your room.
Honesty matters here. The fluid and the anti-nausea medicine have a clear, sensible effect: they correct dehydration and stop vomiting, which is why many people feel noticeably better before the bag is finished. The vitamins are a different story. There is no good evidence that B vitamins or vitamin C shorten a hangover, and nothing in a drip makes your liver process alcohol faster or protects it from damage. Drips are often marketed as an energy boost, but any extra energy people notice comes from being rehydrated and no longer nauseous, not from the vitamins, and fatigue from lost sleep, along with headache and low mood, may persist until you have slept. If a clinic promises a detox or a cure, that is marketing, not medicine.

When a hangover is not a hangover
Alcohol poisoning
Alcohol poisoning happens when blood alcohol is high enough to depress the brain’s control of breathing, temperature and consciousness. Warning signs are confusion, being unable to stay awake, vomiting while drowsy (with a risk of choking), seizures, slow or irregular breathing, pale or bluish skin and a low body temperature. Someone in this state should never be left to sleep it off. Lie them on their side, stay with them and call 1669. A drip in a hotel room is not the right place for this person.
Methanol from counterfeit or home-made spirits
Methanol is a toxic alcohol that can end up in fake branded spirits, cheap cocktail buckets or home-distilled liquor. Thailand and neighbouring countries have seen cases and deaths among both locals and tourists. The danger is the delay: early on it feels like ordinary drunkenness or a hangover, but 12 to 24 hours later people develop blurred vision, a snowstorm or tunnel effect in their sight, sensitivity to light, severe headache, abdominal pain, vomiting and fast, deep breathing. Any change in vision after drinking is an emergency. Go to a hospital emergency department or call 1669 straight away, and tell the doctors what you drank and where, and whether friends drank the same. Hospitals can give antidotes and dialysis, and outcomes are much better with early treatment.
Other look-alikes
Upper tummy pain after drinking can be gastritis or, less often, pancreatitis, which causes severe pain that goes through to the back. Vomiting blood or passing black stools suggests bleeding in the stomach. A fever is never part of a hangover; in Phuket it raises the question of dengue, flu or food poisoning, and needs a proper assessment.

We check blood pressure, pulse, temperature and hydration, and look for any of the warning signs above. When to see a doctor
Most hangovers settle within 24 hours. See a doctor the same day if you cannot keep fluids down for more than 6 to 8 hours, you feel faint when you stand, you have severe or persistent upper abdominal pain, or you have a fever. Prevention and early detection
The most effective prevention is drinking less, but there are practical steps for a night out. Eat a proper meal first, alternate each alcoholic drink with water, and have a glass of oral rehydration solution before bed. In Phuket, drink sealed, branded spirits from reputable bars, be wary of very cheap buckets or unlabelled bottles, and avoid home-made liquor. Watch your friends as well as yourself: early detection of alcohol poisoning or methanol poisoning is usually made by the person next to you.
Avoid ibuprofen and aspirin the morning after, because they can irritate an already inflamed stomach. Paracetamol at the normal dose is generally acceptable for a headache, but do not exceed the label dose after heavy drinking.

A hangover happens after you have sobered up: you are awake, thirsty and miserable. Alcohol poisoning happens while alcohol levels are still high: the person is confused, cannot stay awake, vomits while drowsy, breathes slowly or irregularly, or has a seizure. That is an emergency.
