Rybelsus is an oral tablet containing semaglutide, the same active ingredient as Ozempic. It is approved for type 2 diabetes management and is available on prescription only in Thailand. At approved doses, Rybelsus produces less weight loss than Ozempic, primarily because oral absorption is lower. For travellers and expats in Phuket who need a prescription or a refill, an English-speaking physician at Takecare Clinic Phuket can assess and prescribe on the same visit.
A long-stay guest at a villa in Bang Tao opened his pill organiser one morning and counted three tablets. He had ten days left before flying home. He had been managing type 2 diabetes on Rybelsus for over a year, blood sugar stable, no drama, and now he was quietly calculating whether he could stretch what remained. He could not, and he should not have tried.
Extended-stay visitors and expats across Phuket, from Laguna to Rawai, face this same situation. And it always raises the same questions: what exactly is Rybelsus, why are the dosing rules so specific, and how does it actually compare to Ozempic?

How Rybelsus Works
Semaglutide belongs to a class of drugs called GLP-1 receptor agonists. GLP-1 is a hormone your gut releases after eating. It tells your pancreas to release insulin, slows how fast your stomach empties, and reduces appetite. Semaglutide mimics this hormone. It lowers blood sugar after meals, reduces HbA1c (your three-month average blood sugar), and tends to reduce appetite as a secondary effect.
Rybelsus and Ozempic contain the same molecule. The difference is how they are delivered, and that changes almost everything about how they behave.
Why the 30-Minute Rule Is Not Optional
Proteins are broken down in the stomach. Semaglutide is a protein, so giving it as a tablet should not work. Rybelsus solves this with a compound called salcaprozate sodium, or SNAC. SNAC raises the local pH in a small area of the stomach lining, protects the semaglutide from degradation, and helps it pass into the bloodstream.
According to the Rybelsus prescribing information, oral bioavailability is approximately 1%. That sounds low, and it is, but the dose is calibrated for it. The absorption rules exist because SNAC only works under the right conditions.
Take Rybelsus with the first sip of water in the morning, before anything else. Use no more than half a cup of water, roughly 120 ml. Wait at least 30 minutes before eating, drinking anything else, or taking other oral medications. Food and extra fluid reduce how much semaglutide reaches your bloodstream. This includes blood pressure tablets, thyroid medication, and other diabetes drugs. They all wait. Rybelsus goes first.

Rybelsus Dosing, Start Low, Move Slowly
The prescribing information starts at 3 mg once daily for 30 days. This is for tolerability, not for blood sugar control. After 30 days, the dose increases to 7 mg once daily. If further reduction in blood sugar is needed, it can go up to 14 mg once daily, the maximum approved dose.
Nausea is most common during the early weeks. The slow start is what keeps most patients on the medication long enough to benefit. Moving the dose up too quickly is a common reason patients stop early.
Rybelsus vs Ozempic, An Honest Comparison
| Rybelsus | Ozempic | |
|---|---|---|
| Route | Oral tablet | Subcutaneous injection |
| Dosing frequency | Once daily | Once weekly |
| Approved doses | 3 mg, 7 mg, 14 mg | 0.5 mg, 1 mg, 2 mg |
| Primary approval | Type 2 diabetes | Type 2 diabetes |
| Weight loss approval | Not approved | Not approved |
| Refrigeration required | No | Yes (before first use) |
| HbA1c reduction | Meaningful, dose-dependent | Consistently strong |
At approved doses, Rybelsus produces somewhat less HbA1c reduction and less weight loss than Ozempic. The PIONEER trials and SUSTAIN trials both show meaningful reductions for each, but the injection consistently outperforms the tablet in head-to-head comparisons because bioavailability is substantially higher with the injection.
Neither Rybelsus nor Ozempic is approved for weight loss. Wegovy is a separate product at higher semaglutide doses approved specifically for weight management. If weight loss is your primary goal, clarify with your doctor whether what you have read about is Rybelsus or a higher-dose formulation. They are not the same product.
Rybelsus and Heart Disease Risk
Most patients do not know this part. Rybelsus has received FDA approval to reduce the risk of major cardiovascular events, including heart attack, stroke, and cardiovascular death, in adults with type 2 diabetes who are at high cardiovascular risk, including those with atherosclerotic cardiovascular disease, chronic kidney disease, or both. This approval, granted in October 2025, is based on data from the SOUL cardiovascular outcomes trial. For long-stay expats in Phuket managing both diabetes and cardiovascular risk, this is worth raising with your doctor directly.
Side Effects, What to Expect and When to Act
Nausea is the most common side effect, especially in the first few weeks. It usually improves as the body adjusts. Vomiting, diarrhoea, and reduced appetite are also common early on, all documented in the PIONEER trial programme and generally manageable with the slow titration schedule.
More serious but less common effects include pancreatitis, gallbladder problems, and kidney injury from dehydration caused by vomiting or diarrhoea. If you develop severe abdominal pain that does not pass, stop the medication and get same-day medical assessment. If vomiting or diarrhoea is severe, you can read about diarrhea treatment in Phuket and seek care promptly, as dehydration affects how well your remaining doses absorb.
Rybelsus is not suitable for patients with a personal or family history of medullary thyroid carcinoma, or for patients with multiple endocrine neoplasia syndrome type 2. It is not approved for type 1 diabetes. Discuss your full medical history before starting.
Getting Rybelsus in Phuket
Rybelsus is prescription-only in Thailand. You cannot buy it over the counter at a pharmacy on Bangla Road or at a supermarket in Chalong. Supply at private pharmacies across the island varies, and not all carry it consistently.
If you have run out mid-trip or need a local prescription, a same-day walk-in consultation is the most practical step. At Takecare Clinic Phuket, an English-speaking physician can review your diabetes history, check your current medications, and prescribe where clinically appropriate. Bring your original packaging or a prescription letter from your home doctor. This makes the assessment faster. For patients based on the island long-term, our prescription refill service in Phuket covers ongoing medication continuity without the need to plan around appointments.
If you want to understand more about what Rybelsus is before your visit, you can also speak with an English-speaking doctor in Phuket by telemedicine ahead of coming in.
Travelling on Rybelsus, Storage and What to Do If You Run Out
One practical advantage Rybelsus has over Ozempic for travellers is that it does not require refrigeration. Store tablets below 30°C in the original blister pack, away from moisture and direct sunlight. A room with air conditioning is fine. A beach bag sitting in the Phuket sun at Kata or Surin is not.
If you miss a dose, skip it and take the next tablet as usual the next morning. Do not take two. If you run out and cannot access a prescription quickly, do not simply stop without speaking to a doctor first. Your diabetes management may need bridging support, and stopping abruptly without review is not the right call. Our walk-in clinic in Phuket can see you the same day.
If you are managing type 2 diabetes in Phuket and need a Rybelsus prescription, a medication refill, or a review of your current diabetes treatment, Takecare Clinic Phuket’s English-speaking physicians can assess and prescribe on the same visit. No appointment needed. We see patients from across the island, including Kata, Karon, Bang Tao, Chalong, Rawai, and Phuket Town. Bring your original packaging or a letter from your home doctor to help us help you faster.
