When stitches should come out
Timing is a balance. Stitches left in too long can leave “railroad track” marks across the scar and are harder to remove, while stitches taken out too early can let the wound split open. Areas with a rich blood supply, such as the face and scalp, heal faster; areas under tension or with poorer circulation, such as the legs, feet and skin over joints, need longer. Healing is often slower in people with diabetes, smokers, older adults and anyone taking steroid medicines, and a doctor may leave stitches in a little longer or remove alternate stitches first.
What happens during stitches removal, step by step
Bring any notes from the hospital or clinic that closed your wound, including the date, the number of stitches and whether any are dissolvable. If you have no notes, the doctor assesses the wound directly. We first look at the whole wound for signs that it has knitted together: the edges should be touching, with no gaps, no pus and no spreading redness.
The wound is cleaned with antiseptic. The doctor lifts each stitch gently with fine forceps, cuts it just below the knot on one side and slides it out, so that the part that was outside the skin is never pulled through the wound. Staples are removed with a small tool that bends each staple so it lifts straight out. Most people feel only a small tug. If the wound needs extra support, we apply closure strips, thin adhesive tapes that hold the edges together for a few more days, and a light dressing. Swimming, showering and scar care
Most wounds can be showered gently after the first 24 to 48 hours, then patted dry. Soaking is different: avoid swimming in the sea, pools and hot tubs while stitches are in, because soaking softens the wound edges and seawater and pool water carry bacteria. After removal, wait until the wound is fully sealed, with no scab or open area, before you swim; for most small wounds that is a day or two after removal, and longer for leg wounds. The doctor will tell you what applies to your wound.
New scars burn and darken easily in Phuket’s sun. Keep the scar covered or use a high-factor sunscreen on it for at least several months, because sun protection in the first year is one of the simplest ways to reduce scarring and get a better cosmetic result. Once the skin has fully closed, gentle massage with a plain moisturiser or silicone gel can help the scar flatten and soften.
When to see a doctor
Do not wait for removal day if a stitched wound becomes more painful, swollen or red, leaks pus or cloudy fluid, smells unpleasant, or opens up, or if you develop a fever. These are signs of infection or wound breakdown and need a doctor the same day. Also come in if a stitch has come loose and the edges are separating.
Go to a hospital emergency department, or call 1669, if a wound opens widely and bleeds heavily, if redness spreads quickly with severe pain and fever, or if an abdominal surgical wound opens and you can see tissue underneath.
Prevention and early detection
The best way to protect a stitched wound is to keep it clean and dry, change the dressing if it gets wet, and avoid the beach and pool until the stitches are out and the skin is sealed. Wear loose clothing over the wound, and avoid sports or activities that stretch it. Check the wound each day, and look for early infection: pain that increases after the first two days is often the first warning sign.
