Zepbound contains tirzepatide, the same active ingredient as Mounjaro. The difference is the approved use: Mounjaro is approved for type 2 diabetes, Zepbound for chronic weight management and obstructive sleep apnea in adults with obesity. Both use the same doses and the same weekly injection schedule. If you are already on Zepbound and you are now in Phuket, this guide covers what you need to know, including storage in a tropical climate and where to get physician support on the island.
You packed your Zepbound pens carefully, flew into Phuket International, and then discovered the hotel mini-fridge in Patong either freezes everything solid or barely cools at all. Or your last pen cracked somewhere between Bangkok and the taxi rank. Or you are a long-stay expat in Chalong who has been managing your weight without a regular GP since you moved here, wondering whether you can continue tirzepatide on the island rather than flying home for a prescription review every few months.
This guide covers what Zepbound actually is, who qualifies, what side effects matter, and what the evidence shows. It also covers the parts specific to Phuket: keeping your pen viable in tropical heat, understanding Thai customs rules for injectable medication, and what to do if something goes wrong with your medication or your health while you are here.

What Is Zepbound, and Is It Really Just Mounjaro?
Yes and no. Both contain tirzepatide at identical dose strengths. The molecule and the injection schedule are the same. What differs is the approved indication. Mounjaro was approved by the FDA for type 2 diabetes management. Zepbound was approved for chronic weight management in adults with obesity, and in late 2024, also for moderate-to-severe obstructive sleep apnea in adults with obesity.
In practice, the label on the box and the prescribing rationale differ depending on your diagnosis. Outside the US, availability varies. If you are in Phuket and need clarity on which product applies to your situation, an English-speaking doctor in Phuket can review your history and advise on the most practical path forward.
How Zepbound Works
Tirzepatide acts on two receptors at once: GLP-1 and GIP, both naturally occurring gut hormones. GLP-1 reduces appetite and slows how quickly food leaves your stomach. GIP works alongside it and appears to amplify the overall effect on body weight.
The practical result is that most people eat less because they feel full faster and stay full longer. Blood sugar regulation also improves. You do not need to understand the receptor biology to use the medication, but it does explain why the weight loss effect tends to be larger than with older single-hormone drugs.

Who Can Use Zepbound
The FDA-approved criteria are a body mass index of 30 or above, or a BMI of 27 or above with at least one weight-related health condition. Qualifying conditions include type 2 diabetes, high blood pressure, high cholesterol, obstructive sleep apnea, and cardiovascular disease.
The contraindication list matters more than the BMI threshold. Zepbound should not be used by anyone with a personal or family history of medullary thyroid carcinoma, a rare thyroid cancer, or Multiple Endocrine Neoplasia syndrome type 2. Pregnancy and breastfeeding are also contraindications. A known hypersensitivity to tirzepatide rules it out entirely.
BMI alone does not confirm you qualify. A physician assessment is required, whether you are starting for the first time or continuing a prescription from home. For expats in Phuket who want ongoing management rather than a one-off consultation, a walk-in doctor visit in Phuket is a practical starting point for that review.
Zepbound Dosing Schedule
Zepbound is injected once weekly, under the skin of the abdomen, thigh, or upper arm. The starting dose is 2.5 mg weekly for four weeks. After that, the dose increases in steps, typically every four weeks, up to a maximum approved dose of 15 mg weekly. Your prescribing physician sets the pace based on your tolerance and response.
Rotate your injection site each week. Using the same spot repeatedly can cause changes under the skin that affect how the medication absorbs. If you miss a dose and your next scheduled dose is more than four days away, take it as soon as you remember. If your next dose is within four days, skip the missed dose and resume your normal schedule. Do not double up.
How Effective Is Zepbound
The SURMOUNT-1 trial, the main phase 3 weight loss study, showed that adults on the highest doses of tirzepatide lost significantly more body weight over 72 weeks compared to placebo. In 2025, the SURMOUNT-5 trial reported a direct head-to-head result: tirzepatide outperformed semaglutide on average weight loss at 72 weeks. This is the strongest direct comparison trial published to date.
Effectiveness depends on consistency. Missed doses, poor storage, and stopping early all reduce the result. The SURMOUNT-4 trial found that most patients regained a significant proportion of their lost weight within about a year of stopping tirzepatide. This is not a short course. It works best alongside changes in diet and activity.
Side Effects and Safety
The most common side effects are nausea, diarrhoea, constipation, and vomiting. These are most likely during dose increases and tend to settle as your body adjusts. Smaller meals and avoiding fatty or rich foods during the early weeks helps.
The serious warnings matter more. Pancreatitis can occur. If you develop severe abdominal pain radiating to your back, stop the injection and get seen the same day. Gallbladder disease, including gallstones, has been reported with tirzepatide. If you are also on insulin or a sulfonylurea, tirzepatide increases your risk of low blood sugar, so check with your prescriber before combining them.
The thyroid tumour warning on the label is based on animal data. No causal link in humans has been confirmed, but anyone with a personal or family history of medullary thyroid carcinoma should not use tirzepatide. If you develop a lump in your neck, hoarseness, or difficulty swallowing while on Zepbound, see a doctor the same day. For urgent assessment in Phuket with no scheduled appointment, Takecare Clinic Phuket offers walk-in consultations for exactly this kind of concern.
Travelling With Zepbound in Phuket
Zepbound pens must be kept refrigerated between 2 and 8 degrees Celsius. According to Eli Lilly’s prescribing information, an unrefrigerated pen kept below 30 degrees Celsius can be used for up to 21 days. Phuket’s ambient temperature routinely exceeds 30 degrees, including in the back of a songthaew, on a Kata beach towel, or in a hotel room with an unreliable air conditioner. Do not leave your pen out in tropical heat and assume the 21-day rule applies here.
For air travel, carry your pens in hand luggage, not checked baggage. Airport security can inspect injectable medications. Carry your original pharmacy label, your prescription, and ideally a letter from your prescriber on clinic letterhead. Thai customs allows personal prescription medications for up to 30 days of personal use with appropriate documentation. Keep everything in its original packaging.
If your pen is lost, damaged, or has been stored outside the correct temperature range for an unknown period, do not use it. You cannot simply pick up tirzepatide at a Thai pharmacy counter without a prescription. For expats managing long-term weight treatment on the island, prescription refill support in Phuket at Takecare Clinic Phuket does not require an advance appointment.
